All of the fees, including the set up fees and month to month fees for all of these types of "high risk" merchant accounts are much higher and any business using any "high risk" type accounts should be prepared to pay higher rates, for the duration of use or until the business owner has proven to a bank that their particular business can operate in good standing without the "high risk" factors involved in other businesses selling the same types of products or services.
